Hanns Kuttner, Mary Harrington Naud, Paula Song, Karoline Mortensen, Wendy Zellers, Jacqui Hinchey, Chris Jacobs and, especially, Helen Levy were instrumental in providing me an education in the institutions and existing research on health care and health insurance, and I am grateful for their encouragement and feedback. And I doubt there exists a better mentor, in health economics or any social science, than Catherine McLaughlin, my coauthor on the second chapter of this volume; for taking a chance on an unfunded first-year grad student interested in macro public finance, introducing me not only to health insurance research but also the very people doing that interesting work, and providing invaluable advice on work and life, I am forever in her debt. So much of what I learned about health economics came from my experience as iii a Graduate Student Instructor in the Health Management and Policy Department of the Michigan School of Public Health. I would like to thank Rich Hirth, Edward Norton, Daniel Eisenberg, Catherine McLaughlin, Omar Alshanqeety, and Edward Okeke for immersing me in health institutions and research.
